preceptorship shift #6

I had another good shift at the hospital with my preceptor. I again too the full team of pt. but this time it was 5 pt and we discharged two and admitted one. It was a very busy day and the hospital was packed. First bed was not empty for 15 min until the new pt was there. I felt like I was not on my A game today. I was two steps behind things that needed to be done. But it was good that my RN was so close and willing to help.

With our admits and discharges I was able to complete objective # 3. I was able to give a report to the rehab facility that our pt was going to. It was a good pt to have be my first over the phone report because the rehab home knew this pt very well as she was a long time resident there. I also got to get report on the new admit that came in from the ER. And when he went down to the OR shortly after coming to our floor I was able to call the OR with report. My nurse is really good to let me take control of the reporting for shift change and any other reporting activity. I am able to give and receive report on all of our pts.

I also finished objective # 1; communicate with the interdisciplinary teams. I was able to report about my pts in an interdisciplinary meeting and coordinate the needs of my pts. Also I have on multiple occasions been able to call the Dr. seeing my pt and discuss the plan of care with him or her. And one thing is for sure, it is a lot easier to talk to the Dr. that is standing in front of you than the one on the phone.
